I can easily say that the approach to rhinoplasty success is evolving in both the USA and Europe.
It has been found that extremely narrowed and artistic nasal tip, which has been accepted nice for many years, has negative effects on nasal function and causes an non-natural appearance. Likewise, the curved nose structures, where the nasal bridge is lowered too much, are no longer applied. These non-natural and non-functional nasal shapes can make the person undergo another surgery due to problems in the middle nasal vault and nasal tip over the years.
In Turkey, when we look at people around us or the media – the world of art, we can see that there are many people who are suffering from this problem. Such that, there is a group of unlucky patients who have undergone surgery three or four times but have had to undergo another surgery to breathe by putting aesthetic concerns aside because their nasal congestion continued. I think the success of rhinoplasty can be summarized in one sentence: “The nose, which is natural, is proportionate and beautiful”. If it is understood that you have undergone cosmetic surgery, the success scale of your surgery is five out of ten. What can we do if minor problems develop over the years although the surgery went well?
If the problem is not major, we can make minor limited corrections. Revision surgeries may last 15-20 minutes or an hour. Not everyone will dare to have a second surgery. I usually try to correct the humps, pits, and arches on the nasal back or contours via the injection of hyaluronic acid which is a filling material. The purpose of filling procedure is not to achieve a perfect profile or appearance but to create small units that make the defects invisible. Anesthesia is not required for filling injection but I perform this after applying local anesthetic cream. We can see the result at that moment. It should not be forgotten that unless we use a permanent filler, these substances melt after six months and can be repeated continuously.
Although a rare allergic reaction and infection have been reported, I have never received such complaints from any of my patients.
Nowadays, rhinoplasty is not only done for having a beautiful nose. There are many functional problems that medically require rhinoplasty. Extremely reduced noses are not natural and not considered beautiful. Furthermore, nasal congestion is likely to develop in these patients because of the deterioration of bone-cartilage stability. If you think that “I can easily get the nose that I want with rhinoplasty”, you are in the wrong way. Please note that with this surgical procedure, we are able to make changes to the extent allowed by the cartilage and bone structure of the nose. I think that special analysis should be performed for each patient and a characteristic nose that matches the face should be planned. A proportioned nose that looks natural, matches facial structures, and breaths easily is the perfect nose. Nose aesthetics should not be considered as only a nose reduction. It is a surgical procedure in which certain parts are enlarged depending on the structure of the nose and the nose is made proportional to the face and forehead. We call this new concept as a “balanced nose concept”, which is necessary for a more beautiful and natural nose with no functional problems.
